Felton’s Senior Division specializes in a full continuum of geriatric mental health outpatient programs and a range of aging services programs, including workforce development, economic security, case management, advocacy and a number of programs addressing social isolation and the digital divide. The Division is also very active with county, city, and community partners for the development of new service areas and policy recommendations.

Program Description
The Senior Division Older Adult Service Team (OAST) joins Felton’s Deaf Community Counseling Services (DCCS) and Early Psychosis program in providing services to the residents of Alameda County, under contract by Alameda County Behavioral Health Care Services. The OAST team is the county’s primary community-based specialist in mental health services meeting the complex needs of older adults with serious mental illness (SMI). OAST supports client recovery and wellness through a person-centered approach that is culturally-responsive, trauma-informed, and considers the unique bio-psycho-social needs of older adults. The Program Manger or Director leads the treatment team, which includes Clinical Case Managers (LCSW, MFT, ASW, MFTi), Community Specialists (who provide peer support), medical staff (Psychiatrist, Psychiatric Nurse Practitioners), and an administrative staff person.

Job Description
This Program Manager/Director provides supervision to Clinical Case Managers and Community Specialists, facilitates team meetings including collaboration with medical and administrative support staff, coordinates referrals and intake, monitors charting compliance, and ensures a welcoming environment for clients. This position is also responsible for meeting all program requirements of the contract. Program Managers participate in the Senior Division management team, which coordinates program implementation and development, and participate in agency-wide management meetings. Other duties may also be assigned.

Program Specific Responsibilities
  • Manage the Senior Division OAST program under the supervision of the Senior Division Director.
  • Provide programmatic and administrative supervision to Clinical Case Managers and Community Specialists.
  • Coordinate crisis management with staff team and facilitate clinical problem solving as needed.
  • Support the operation of the medical team, and coordinate multi-disciplinary team conferencing between program staff and medical staff.
  • Coordinate referrals, intake, and caseload assignments, and facilitate team meetings.
  • Monitor staff charting to ensure compliance with the Alameda County BHCS contract.
  • Develop systems and operations to meet program goals, including systems for step-down of services and referrals of clients who graduate from programs to other servicing agencies.
  • Provide direct services, including facilitation of intervention and wellness groups, as needed.
  • Continue to build the OAST program and team, as needed, with support of the Senior Division management staff.
  • Work with Felton Division of Training and Research to strengthen implementation of evidence-based practices and develop research tools to measure effectiveness of geriatric outpatient treatment model.
  • Maintain and develop working relationships with Alameda County BHCS and community partners including the Area Agency on Aging, In-Home Supportive Services, Adult Protective Services, and other stakeholders.
  • Additional duties as needed.
Minimum Qualifications
  • MA/MS in Social Work or Psychology and registered with CA BBS, with licensed preferred (LCSW, LMFT, LPCC).
  • Minimum 4 years experience providing direct service to clients with serious mental illness; direct service to older adults preferred.
  • Experience in program management, program development, and staff supervision.
  • Direct knowledge of Medi-Cal billing codes and requirements and DSM-V diagnoses.
  • Strong facility with electronic medical record systems.
  • Knowledge of Alameda County behavioral health and community resources preferred; knowledge of services for older adults preferred.
  • Experience in managing projects, making independent decisions, and problem solving.
  • Exceptional written and verbal communication skills, including public speaking.
  • Experience and knowledge in the field of aging preferred.
  • Outstanding organizational skills and attention to details.
  • Proven experience as a team player with a welcoming attitude.
  • Personal practice of cultural humility and ability to promote cultural humility among staff to provide quality services to clients from a wide range of background. Bilingual/Bicultural ability a plus.
