Full Job Description

Well known, growing non profit in the San Mateo County area is hiring for a Director of Case Management for their step down program. This program provides personalized rehabilitation services to people who need support while they develop and work towards accomplishing their life goals. Services are provided in the community and based upon an individualized treatment plan that focuses on strengths and abilities. The program helps individuals maintain housing by helping to develop daily living skills.

Responsibilities:

  • Provides leadership to staff and managing the day-to-day operations of the program, ensuring that clients receive the highest quality of care and that staff are properly trained and oriented to their jobs.
  • Ensure case managers maintain at least a 65% billable service time to total work time percentage

productivity level.

  • Oversee the development of client treatment plans and other charting documentation, including

initial assessments, admission and discharge summaries, and progress notes.

  • Collaborate with QA to make updates to all forms to keep in compliance with Medi-Cal and County

standards.

  • Ensure case managers support the development and attainment of client rehabilitation goals,

including securing of housing and employment, development of a support system, increasing
socialization skills, participating in recreational activities, etc.

  • Evaluate need and eligibility and provide direction to case managers around client entitlement

benefits and obtaining them.

  • Ensure cleanliness and maintenance of client living environments, providing direction and guidance

as necessary in order to meet stated standards; Train, model and assist Case Managers in teaching
activities of daily living, such as meal planning and preparation, personal hygiene, and budgeting;


Qualifications:

  • Master’s Degree and registered with California Board of Behavioral Sciences (BBS) as AMFT, ASW or

APCC required. Licensed as LMFT, LCSW or LPCC preferred

  • Minimum two years of demonstrated work experience providing services to SMI/DD preferred.

Demonstrated experience in a program serving homeless individuals and individuals with co-occurring
disorders strongly preferred.

  • Minimum one year demonstrated experience providing program management, staff supervision, and

leading multidisciplinary teams in a mental health related field is strongly preferred.

  • Demonstrated knowledge of CARF accreditation process.
